If you live within our practice boundary and would like to become a patient, please register using our online form.

Patient Registration

When you register with the practice, you will be allocated to one of our doctors who will be responsible for your care.

All new patients who are adults should request a new patient check with our healthcare assistants. This will enable us to check blood pressures, offer health advice and take details of any repeat medications that you are taking. You may need to see a clinician if you need any prescriptions.

You will be given a container when you register at the practice, please bring a urine specimen with you to your appointment.

If you are registering at our Reepham practice your registered doctor will be a Reepham doctor, and if you are registering at our Aylsham practice your registered doctor will be an Aylsham doctor.
